Consent to the Processing of Personal Data

In accordance with the requirements of Article 9 of Federal Law No. 152-FZ “On Personal Data” of 27 July 2006, I, freely, by my own will, and in my own interest, give to FLOK LLC (OGRN 1067746567976, INN 7724576641, address: 115201, Moscow, Kashirskoye Highway, 22, apt. 3, bldg. 2, premises 2,3), hereinafter referred to as the “Operator”, acting through its General Director Igor Petrovich Efimov on the basis of the Charter, my consent to any action (operation) or set of actions (operations) performed with or without the use of automation tools, including the collection, recording, systematization, accumulation, storage), clarification (updating, modification), extraction, use, transfer (dissemination, provision, access), depersonalization, blocking, deletion, destruction) of the following personal data of mine:

1. List of personal data transferred:

1.1. Surname, first name, patronymic; 1.2. Email address; 1.3. Telephone number; 1.4. Payment card data (payment card number, name and surname of the cardholder, date of issue, card expiry date), other payment details; 1.5. Date and place of birth, gender, and age; 1.6. Identity document data (series, number, and date of issue, issuing authority, validity period); 1.7. Residential and registration addresses; 1.8. Other data voluntarily provided to the Operator (including information automatically collected through third-party services).

2. Purposes of providing personal data

2.1. Conclusion of contracts, settlement of mutual accounts. 2.2. Performance of the contract between me and the Operator. 2.3. Processing and receipt of payments, confirmation of tax or tax benefits, payment disputes. 2.4. Carrying out informational mailings. 3. This consent is valid for the period until the expiry of the retention periods for the relevant information or documents containing such information, determined in accordance with the legislation of the Russian Federation. 4. Access to personal data may be granted to employees, partners, and other counterparties of the Operator who require such access in connection with the performance of their official and/or contractual obligations, both with me directly and with the Operator. 5. The Operator has the right to transfer my personal data to bodies of inquiry and investigation, and other authorized bodies, on the grounds provided for by the current legislation of the Russian Federation. 6. The Operator does not transfer personal data to third parties, except upon the request of a court or another authorized state body within the framework of the procedure established by law.

7. I hereby confirm that:

7.1. The Operator has explained the consequences of failing to provide personal data in cases where the provision of such data is mandatory in accordance with federal law. 7.2. I have been informed of the possibility of withdrawing consent on the basis of the provisions of Federal Law No. 152-FZ “On Personal Data” of 27.07.2006 by means of a personal request or by sending a written request (including in the form of an electronic document signed with a simple electronic signature or an enhanced qualified electronic signature). 7.3. I have been informed of other rights and obligations arising from this Consent. 8. The consent may be withdrawn by sending a statement in free form to the Operator’s address by any means that allows the fact of its receipt by the addressee to be unambiguously established and the identity of the sender to be determined. 9. The moment of completing the relevant field (clicking the relevant button) in the electronic form shall be considered the date and time of the formation, confirmation, and submission of the consent.
